Community inclusion, independence, decision-making, and personal choice should be promoted through these supports, with a focus on individual needs.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">RESPONSIBILITIES AND DUTIES<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Support clients to participate in age-appropriate activities and tasks including employment, in-home support and community access.<\/li>\n\n\n\n<li>Support clients to access activities and outings in the community.<\/li>\n\n\n\n<li>Provide direct care assistance to each client in accordance with their service plan and as directed.<\/li>\n\n\n\n<li>Work in accordance with the medication administration policy, work health and safety procedures and client rights policies including privacy and confidentiality.<\/li>\n\n\n\n<li>Monitor the safety and well-being of the clients and report any concerns to the Manager.<\/li>\n\n\n\n<li>Provide 1: 1 or small group training and support in response to individual client needs.<\/li>\n\n\n\n<li>Ensure completion of appropriate documentation.\u00a0 Relevant information on support provided is to be recorded in client notes and communication books as required.<\/li>\n\n\n\n<li>Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.<\/li>\n\n\n\n<li>Work with clients, family\/ carers, relevant case managers and other professionals to support the client&#8217;s individual goals.<\/li>\n\n\n\n<li>In conjunction with other members of staff assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals.<\/li>\n\n\n\n<li>Ensure that all programming undertaken is appropriately maintained, documented and reviewed.<\/li>\n\n\n\n<li>Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress.<\/li>\n\n\n\n<li>Liaise extensively with other staff to ensure the overall smooth and consistent running of the service.<\/li>\n\n\n\n<li>Report regularly to the manager<\/li>\n\n\n\n<li>Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes and policy review.<\/li>\n\n\n\n<li>Provide backup and support to other staff as necessary.<\/li>\n\n\n\n<li>Participate in Performance Development meetings to identify ongoing training and support opportunities.<\/li>\n\n\n\n<li>Notify the service if you are unable to work your rostered shifts, with as much notice as possible.<\/li>\n\n\n\n<li>Contribute to Case reviews with Employment Consultants and Facilitators.<\/li>\n\n\n\n<li>Clients receive appropriate levels of support essential to promote and maintain independence and participate in age-appropriate activities.<\/li>\n\n\n\n<li>All clients receive a high level of care and support as per their Lifestyle Service Plan (LSP).<\/li>\n\n\n\n<li>Personal care is provided to clients in a caring and supportive way.<\/li>\n\n\n\n<li>Documented evidence demonstrates that service procedures are followed.<\/li>\n\n\n\n<li>All policies and procedures are followed.<\/li>\n\n\n\n<li>Records demonstrate that clients have received correct medical and dietary intakes as prescribed by medical practitioners and as per service policy and procedures.<\/li>\n\n\n\n<li>Client files are updated and reviewed and health care and behavior support plans are in place as required.<\/li>\n\n\n\n<li>Documentation of annual Lifestyle Service Plans and reviews are sighted by the Manager.<\/li>\n\n\n\n<li>Daily documentation completed (i.e. diaries and daily statistics).<\/li>\n\n\n\n<li>A high level of communication with the Manager is maintained including advising of any potential risks to clients or the organisation.<\/li>\n\n\n\n<li>Induction, WHS, Manual Handling, Medication and Infection Control training are undertaken on commencement.<\/li>\n\n\n\n<li>Documentation is completed and maintained as per procedures.<\/li>\n\n\n\n<li>Sufficient notice (as much notice as possible) is provided for all rostered shifts you become unable to work.<\/li>\n\n\n\n<li>Employee code of conduct is adhered to.<\/li>\n<\/ul>\n\n\n\n<div class=\"wp-block-group is-layout-constrained wp-block-group-is-layout-constrained\">\n<div class=\"wp-block-group is-layout-constrained wp-block-group-is-layout-constrained\">\n<h5 class=\"wp-block-heading\">PERSONAL QUALITIES<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Being adaptable to changing circumstances and being able to prioritise work.<\/li>\n\n\n\n<li>Client focused, and person centered<\/li>\n\n\n\n<li>Commitment to social and inclusion and valuing difference and diversity.<\/li>\n\n\n\n<li>Staff that deliberately breach this dress code may be subject to disciplinary action.<\/li>\n\n\n\n<li>Staff must comply with Equicare Support Services\u2019 Privacy and Confidentiality Policy and Procedure regarding the collection, storage, use of personal and health information.<\/li>\n\n\n\n<li>Equicare Support Services does not accept any form of aggressive, threatening or abusive behaviour towards its staff by other staff, clients or stakeholders.<\/li>\n\n\n\n<li>Internet and email are provided to staff members is for genuine work-related purposes.<\/li>\n\n\n\n<li>Use of Equicare Support Services communication and information devices are for work purposes only.<\/li>\n\n\n\n<li>Staff must not share their passwords with another staff
